An exciting opportunity has arisen for an experienced nurse to join our Inclusion Health Team.

The postholder will work with the current team to assess and support all Inclusion Health patients as inpatients, in the Emergency Department or Rough sleeping on UCLH premises with immediate health needs as well as access to longer term support and housing.


This may include GP registration and access to further healthcare, substance misuse services and other homeless and health support services internally and externally to UCLH.


We are seeking a flexible, innovative, and motivated individual who will provide effective wraparound, trauma-informed health interventions for Inclusion Health patients at UCLH.

The postholder will be required to work closely with multiple stakeholders both internal and external to continue developing effective collaborations to provide care to the Inclusion Health patients across NCL.


These include the existing members of the Inclusion Health team, the wider Integrated Care Operations teams including the Integrated Discharge Service, and the Head of Integrated Care Operations at UCLH, substance misuse services, street outreach teams, and other outreach homeless services across the integrated care system.


The post holder will need an understanding and appreciation of the barriers faced by Inclusion Health patients, especially homeless and rough sleeping populations, and the ability to adapt practice to best meet the needs of this population in a psychologically informed way.

The post holder will also need a thorough understanding of acute inpatient settings and patient flow to help support the safe timely discharge of clients.

This post is fixed term for 2 years due to grant funding.
