A new and exciting key post has arisen for an enthusiastic individual to join the All Age Continuing Care team in Cheshire.

We are looking for someone with Healthcare experience, who is willing to learn and develop their skills in All Age Continuing Healthcare in Cheshire, and we will support you to work towards a Level 5 Continuing Healthcare Practitioner qualification (Apprentice).


All Age Continuing Healthcare is part of the Nursing and Care Directorate, and you will play a key role in helping our award wining service, deliver key aspects of The National Framework for Continuing Healthcare.

Cheshire East/West Full time (37.5hrs), Monday-Friday.

Working within the remit of the National Framework for Continuing Healthcare, the post holder will deliver an effective and competent level of service and consistency deliver a client-focused service.

The post holder will be required to work under their own initiative as well as working as part of the wider team and will demonstrate a willingness to help others and be a key member of the team with responsibility for specifically allocated tasks.

The post holder will be expected to follow set processes, prioritise their own workload and demonstrate initiative in order to fulfil the requirements of the post.

Excellent interpersonal skills and communication skills are essential plus an ability to deal with highly sensitive and confidential information.


The post holder will assist and support in the facilitation of multi-disciplinary assessments and patient reviews in line with standard operating policy and procedures and ensuring The National Framework for NHS Continuing Healthcare and funded-Nursing Care is implemented and complied with according to the principles and processes.

The service operates Monday to Friday between the hours of 9am-5pm.

The orgaisation has a strong focus on the wellbeing of its staff and is able to offer regular 1-1's, both formal and informal wellbeing activities and an employee assistance programme.

The Service works on a hybrid model working at home and at offices based in Chester, Macclesfield and Nantwich.

  • Working within a team of Continuing Healthcare Assessors, ensuring a highquality service is delivered, supporting and mentoring and supporting staff to achieve continuous improvement
  • Supporting the Continuing Healthcare team to ensure that workstream plans are fulfilled and supervised effectively to ensure successful delivery
  • Be responsible for managing own workload as delegated by team manager and Independent Assessor
  • Maintain links with academic agency and report any difficulties with completing course work
  • Report any difficulties with completing the delegated tasks
  • Support the Continuing Healthcare team to ensure assessments and reviews are completed in a timely manner
  • Maintain accurate records on team's database system
  • Attend team meeting including team supervision sessions for clinical support; identifying and sharing good practice
  • Maintain good working relationships with CHC team and other ICB colleagues
  • Work in partnership with Local Authorities' colleagues, and other allied professional in the local community teams to ensure service delivery in relation NHS Continuing Healthcare
- responsible for screening and assessing individuals for eligibility to continuing healthcare packages in line with the National Framework for Continuing Healthcare, related legislation and policy. To support this assessment work, they undertake analysis, interpretation and evaluation of evidence from multiple health and social care professionals caring for the individual.

  • Actively supporting the Continuing Healthcare team in working towards goals and objectives
  • Communicating information, risks, issues and dependencies, including briefings and reports
  • Maintaining uptodate knowledge of legislation, policies and guidance
  • Maintaining constructive relationships with a broad range of internal and external stakeholders
  • Working closely with care providers involved in delivery of the service
  • Delivering against objectives, achieving quality outcomes, prioritising own workload and working to tight deadlines
  • Ensuring own compliance with all statutory and mandatory training
  • Fostering transparency and a personcentred approach with patients, families and representatives
  • Carrying out other appropriate delegated duties as required
