Description

The post holder will be required to work with the Patient flow team to support with Safe patient discharge from Acute Mental Health inpatient wards.

The Patient flow team liaises with all services and agencies to ensure that patient discharges are timely and any obstacles to this are resolved.


The post holder will have delegated responsibility for some aspects of the delivery of the service, under the supervision of qualified practitioners.


The post holder will attend the escalation meetings and ensure that the patient flow spreadsheet is updated from these with the accurate information discussed.

They will adhere to all relevant trust polices will and work as part of a team managing their own time and organise workload balancing competing and conflicting priorities.


The post holder may be required to liaise with various teams, agencies, patients and carers under the supervision of a qualified practitioner.


The post is designed to deliver a dual function of clinical and administrative support to the team and will include responsibilities in both clinical and non-clinical function.


The post holder and team will be required to work to the organisation's agile working policy and to uphold the vision and values of the organisation.

The post holder will be required to undertake delegated responsibilities as required.
