Hampshire County Council are seeking a Case Worker in their Adults Health and Care - Multi Agency Safeguarding Hub (MASH) Team.

A Non Qualfied role Based in Fareham with Hybrid working. An excellent oppourtinity for someone newly qualified as a Social worker or someone completeing their degree (but not essential as long as has relevant experience )

Tasks and responsibilities:

  • Respond to telephone and online referral of Safeguarding Concerns.
  • Identify individuals' level of needs and support available, involving them as fully as possible to support Making Safeguarding Personal
  • Adopt the strengthsbased approach in all role responsibilities
  • Develop and maintain specialist knowledge in relation to needs and resources in order to support Adult Social Work, Mental Health, Learning Disability, Sensory Service and Occupational Therapy.
  • Writing up concise reports on client's files to effectively communicate to Front line staff such as Social Workers
  • Follow up calls to confirm clients have received best possible service available to them?
  • Record Compliments and complaints as per HCC policies
  • Continuously seek and communicate Business Improvements
  • Maintain excellent working relationships with all colleagues at all levels
  • Provide and receive feedback for development
  • Being available to the customer at the correct shift times, unless agreed otherwise.
  • Attending all training in a positive manner to ensure knowledge is kept up to date and taking responsibility to proactively keep selfinformed of relevant policies and legislation. Show flexibility and adaptability to changing processes and cultures.
  • When making decisions show an understanding of the legal frameworks and financial implications and risks that the decision will have.
  • Working to support Adult Services employees and management to deliver on the service level agreement

  • Safeguarding Concerns
  • Professional referrals
  • Referrals from individuals and families
A knowledge of wider Adults Health and Care Business in the following areas is required:

  • Wellbeing checks
  • Strengths based screening and resolution
  • Carers Assessment
  • Individual Assessment of Care and Support Needs


  • Finance questions

  • Paying for Care, Direct Payments Deferred Payments, Self-Funding Capital Depletes, Financial Eligibility Questions.
  • Provide information and signpost Care Choice Telecare, Pendant Alarms, Meals on Wheels
  • Going into and Coming out of Hospital
  • Sensory Services Enquiries, Screenings and Equipment advice
  • Palliative Care
  • Mental Health


  • Reablement

  • OT Equipment Fast Track
  • OT A Fast Track
  • OT assessment
  • Learning disabilities
  • Transport queries
