Come and join our new and growing Community Mental Health Team as a
_Community Mental Health Practitioner._
Based in one of the locality hubs you'll be part of a multi-disciplinary team to deliver joined up care for people either in or as close to their own homes as possible.
As a
_Community Mental Health Practitioner_, you'll provide specialist community mental health services for adults, supporting them and their carers to help identify, develop and improve their mental and related physical wellbeing.
All you need is:
- An understanding of mental health issues, services and current legislation.
- Experience of undertaking assessments to inform care plans and interventions.
- The ability to engage with a range of providers to meet the wider needs of clients' care.
- Access to a car for work purposes and hold a full current UK driving licence.
