Integrated Complex Assessment team is responsible for the placing, assessing and tracking of patients requiring a discharge to asses (D2A) bed based service within a timely manner relieving the pressures to the acute hospitals.


The Administrator is required to effectively provide administrative support to assist in the delivery of safe and effective patient transfer and tracking within Discharge to Assess Services.


The post holder should be able to prioritise their own workload to ensure effectiveness and efficiency in delivering objectives whilst working with mínimal supervision.

Being proactive and able to demonstrate good organisational skills and excellent communication skills both in verbal and written correspondence.


The role will involve close working with health and social care staff and an understanding of community Home First services alongside knowledge of the Care Act 2014 and the principles of Discharge to Assess (D2A) would be desirable.

The role is varied and requires confidentiality, initiative, flexibility and capability of working under pressure and to deadlines.

Main duties and responsibilities

  • To effectively track the patients journey from referral to discharge in a timely manner using in house IT systems CDT and excel.
  • Develop and maintain strong relationships with all partners and colleague to support timely updates and flow through discharge to assess beds.
  • To be able to work closely and at pace, assisting other members of the administrative team to meet deadlines, sharing workload and providing cover for absences where necessary
  • To provide a comprehensive administrative support service to the ICAT Team by ensuring in house system is up to date, signposting to relevant services and data collection
  • Use CDT to maintain and monitor patient flow for audit purposes
  • To participate in service improvement programmes appropriate to the banding of the post
  • To provide cover for other Administration colleagues in the event of absences
  • Co-ordination of Multi-disciplinary meetings via Teams for weekly length of stay reviews with D2A providers
  • Minute taking as required in family meetings or where needed.
  • Ordering, checking and issuing of supplies and stationery using the electronic ordering system in accordance with financial procedures/regulations.
  • Undertakes basic audits, surveys related to project.
  • Flexible approach - demonstrating ability to respond positively to changes in allocation of work at short notice to set deadlines.
  • May need to travel across D2A footprint to provide Administration support
  • Undertake yearly staff survey
