Mental Health Caseworker
The recovery support worker will be part of a team providing effective, responsive, high-quality, resident focused support services to address the presenting needs of our Residents.
You will be responsible for leading and coordinating the support for allocatedclients by providing practical and personal support to residents in a way that is approachable, engaging, empathetic and flexible.
You will be responsible for formulating comprehensive needs assessments and risk assessments resulting in an agreed support plan.Ø To carry out comprehensive needs assessments in conjunction with other team members with prospective residents. Following the interview, this will involve comprehensive documentation and contribute to referral decisions
Ø To assess factors which could impact on the safety of residents or others, and to develop high quality and comprehensive risk management plan with the resident and any other services involved in delivering relevant support to an individual.
Ø To identify and work with a range of agencies to ensure all residents have access to relevant services including both statutory and non-statutory support and community resources.
Ø Working in partnership with residents and others involved in their support, to translate needs assessments into support and action plans which are person centred, outcome focused, and able to demonstrate clear and appropriate goals and actions in orderto enable residents to realise their aspirations.
This includes acting as a central coordination point for external support services and to increase or taper support in line with presenting need.
Ø To accompany clients to appointments with other services where appropriate or liaise with a Support Assistant to ensure that the resident is supported appropriately.
Ø To complete detailed and thorough sign-ups to ensure that new residents understand the rights and responsibilities of both themselves and under their tenure agreements and to assist with all areas of their move in to ensure new residents are made to feelcomfortable and are given information concerning their tenure agreement, the local area and other services.
Ø To challenge disruptive or unacceptable behaviour in a sympathetic and assertive way, balancing the needs of the individual with responsibilities for the safety of the service
Ø To take personal responsibility for feeding back to colleagues/managers any information or comments that would contribute to ensuring the service is relevant and responsive.
Ø To keep abreast of current housing legislation, welfare benefit legislation and other matters relevant to successful support and resettlement of clients.
Ø To attend shift handover meetings, weekly Team Meetings, team review days and other meetings as agreed with the Unit.
HSC1 Essential Requirements:
- Educated to GCSE level or equivalent
- Experience in helping people to identify personal goals and supporting them through a process of change
- Experience of dealing with complex and difficult situations in relation to people.
- A sound understanding of the issues faced by people who are homeless or vulnerably housed and the difficulties they can experience in accessing services that are fully responsive to their needs
- Experience of liaising with and/or coordinating several individuals and/or agencies to achieve effective outcomes.
