Description

The NHS Long Term Plan calls for health care to make personalised care business as usual across the health and care system as one of the five major, practical changes needed to achieve the new NHS service model.

Personalised care means people have more choice and control over the way their care is planned and delivered based on 'what matters to them' and their individual diverse strengths, needs and preferences.

This post holder will:

Empower patients to live well through better conversations, selfmanagement education, care coordination and facilitating peer support
Influence staff to embed personalised care planning and shared decision making in practice
Plan and organise a broad range of complex programmes and contribute to the sustainability long term
Contribute to the development of the personalised care strategy for the Trust
Gather data and analyse benchmarking information for personalised care trends and identify opportunities for transformation
Undertake observations of clinical services and pathways to identify opportunities for transformation/improvement
Facilitate the implementation of transformation and pathway redesign identified through improvement activity and feedback
