Description

The role will include assessment of patients with Lower GI conditions and the development of individualised management plans in conjunction with the Endoscopy guidelines.

This will include participating in telephone/face to face assessment clinics for triage and assessment of fast track/routine and Therapeutic referrals.

This also involves the adjustment of medication in accordance with national guidelines (For example Diabetes and Anti Coagulation). Monitoring and requesting of patient's bloods and acting on abnormal findings. Liaising with other MDT members for the prescribing of extended bowel preparation. The provision of education to the Endoscopy patient is essential and the post holder will have excellent communication skills.

The post holder should ensure the needs of all Endoscopy patients are recognised and met by the assessment, planning, implementation, and evaluation of care.
To participate in MDT with the Endoscopy team and promote care management to ensure the patients journey is seamless.
Act as a point of liaison for multi-disciplinary and multi-agency working in order to achieve the best possible outcome for patient care. Communicating with other MDT Members regarding Vulnerable patients (For example liasing with Learning Disability Nurses to help facilitate and support the patient during their care within Endoscopy.
To provide advice and support to patients undergoing Lower GI Procedures.
To provide advice and support to ward staff regarding all aspects of Endsocopy procedure if patient needs admission.
To participate in teaching programmes to educate and develop other staff/patients and carers on Endoscopy Procedures/Medication Advice as required. To ensure the sharing of clinical and theoretical knowledge to promote research-based practice.
Maintain the Trust's aims and objectives within the clinical setting.
