Job title - Dementia Coordinator

Responsible to:
Dementia Co-ordinator Team Leader

Salary:
£22,295 per annum for a 35-hour week

(7 hours per day plus 30-minute unpaid lunch break each day, Mon-Fri)

Base:
Herne Bay

20 Days Annual Leave + Bank Holidays

ADVERT FOR DEMENTIA COORDINATORS

Would you have the skills and motivation to support people affected by dementia to access the knowledge, information, advice, treatments, support and care they need to remain as independent as possible within their families and communities?


Working with a group of local GP surgeries in Herne Bay our Dementia Coordinators are there to support people along the dementia journey; from before they get a diagnosis right through to end of life.

They provide support to people affected by dementia, helping them to navigate the health and social care system and manage the many challenges of living with a progressive condition.


Salary:
£22,295.00 per year
