An exciting opportunity has arisen to join the Swindon Community Virtual Ward. _A virtual ward is a safe and efficient alternative to NHS bedded care that is enabled by technology.

Virtual wards support patients who would otherwise be in hospital to receive the acute care, monitoring and support they need in the place they call home.

This includes either preventing avoidable admissions into hospital or supporting early discharge out of hospital. _

The Virtual Ward will provide senior clinician led care for a variety of patients including those with respiratory conditions and Frailty.

This care is provided to them in the comfort of their usual place of residence using advanced personal remote monitoring equipment, enabling them to safely leave hospital sooner or not come into hospital at all.

The virtual ward has multi-professional input from consultants, specialist nurses, advanced care practitioners and a wealth of other health and social care staff.

The Swindon Community Virtual Ward Team provides holistic person-centred care with the aim of preventing unnecessary hospital admission or supporting early discharge in a variety of ways.
