This is a full-time permanent post.


As part the Unscheduled Care Team within the whole system of Community Mental Health Services the post holder will work closely with all disciplines/agencies.

The post
holder will under the direction of qualified staff to provide a service for individuals with complex mental health, physical and social care needs who present in crisis who may

require additional home treatment/care intervention over a 24hr period, also providing a specialist mental health service into the Mental Health Assessment Units, Emergency Departments and inpatients services within the General Hospital setting.

The post holder will be expected to take part in a wide range of duties as directed by a qualified member of staff, duties will include offering telephone triage and crisis
support to patients and carers experiencing mental health crisis, engaging with health, social care and other professional agencies to support the patient pathway during
unscheduled care.
The service provides assessment and access to mental health inpatient beds over a 24hrs period throughout GG&C. This includes arranging hospital admission for a patient should this be required.

Due to the rapid response nature of Unscheduled Care, the post-holder will be expected to manage a range of duties including participating in assessment procedures, accurate record keeping and participation in team meetings.

The service operates over a 24hr period and has a number of bases across NHS GG&C.
