Each Youth Practitioner covers a Geographical area within the Co Durham 0-25 Family Health Service and are very much part of the locality and County wide youth practitioner teams.

The Youth Practitioners work with young people aged 11-19 years old (25 for SEND) around Healthy lifestyles, with a strong focus on personal, social, emotional, cultural and educational development.

They work with vulnerable young people both one to one and in groups delivering positive activities that develop self-esteem, confidence and resilience.

Supporting them to develop healthy relationships and build Life skills.
