Description

The post holder will be responsible for the coordination, planning, implementation and evaluation of the diabetes education as well as providing clinical support to the patients with Harrow locality.

The postholder will be working collaboratively with GPs within Harrow PCN and the Community Diabetes Specialist Service.

The post holder will be a registered nurse and work in line with the requirements set out in the NMC code which require all registered nurses to prioritise people; practice effectively; preserve safety; promote professionalism and trust.
