Direct Medics is a Registered Supplier to the Northern Ireland Health and Social Care Trust Framework.

We are currently working with Discharge Hub Assistants to help them find new and exciting opportunities with the Trusts here in Northern Ireland.

As such, we are now recruiting on behalf of the Belfast Trust for a Band 4 Community Discharge Hub Assistant/Social Work Hub Assistant to work in the Belfast Trust sites (Royal Victoria Hospital/BHSCT sites The purpose of The Community Discharge & Social Work Hub is to facilitate safe discharge on the appropriate pathway for patients who have been declared medically fit within a time frame of 48 hours. This role will be full time (37.5 hours) Monday - Friday for 6 months initially.

Requirements:

Five GCSE's including English Language (Grades A-C) or equivalent or NVQ Level 3 in Care or equivalent
A minimum of 2 years paid or voluntary experience working with Older People and or people with physical health and disabilities in either a social care or hospital setting in a caring capacity
Full UK drivers License
Right to Work in the UK

Job Types:
Full-time, Temporary contract

Contract length: 6 months

Salary:
£13.79-£24.50 per hour

Expected hours: 37.5 per week
