Care Leavers Outreach Worker needed with Worcestershire County Council

Description:

Working with children, young people and families as part of the Turnaround Early Intervention Programmes
To act as the professional lead for a caseload of Turnaround referrals, sourcing and signposting to appropriate agencies where required, within the family's residing area
Responsible for continual assessing and reviewing of strengths, needs and risks as part of the Turnaround Early Intervention Project
Responsible for completing or overseeing planned activities or interventions with children, young people and families

What is required?

Ability to travel drivers license and access to a vehicle
Demonstrable experience of working with vulnerable children, young people and families
Demonstrable experience working in a community setting
Demonstrable experience of managing a caseload/casework
Evidence of providing structured therapeutic interventions
Experience of assessing, developing, implementing and evaluating interventions
Level 5 in education in Health, Social Care or equivalent experience
Trained in providing structured CBT psychological therapy
