We have an exciting opportunity to join the GMMH Rehab division as an Advanced clinical practitioner.

The post holder is required to work across the whole division with some focus on working with our partner organisation, Elysium at Braeburn house in Salford to support MDT / partnership working.

The post holder will support the Rehab Quality matron and Operational manager as well as the wider team to deliver clinical leadership, ensuring the delivery of safe, effective high quality clinical care and consistent achievement of care standards across the division.

The Advanced Practitioner will be responsible for


Assessment, formulation and prescribing of care planning including discharge planning within agreed protocols and will advise the multi-disciplinary team on expert, safe and effective models of care and treatment.

Providing clinical leadership and management leading to the provision of safe, quality and effective care.


Leading on the Physical Healthcare agenda for the Division and will support the improvement of workforce knowledge through inpatient education and training programme.


Educating and facilitating all other professions within Rehab services to meet the care needs of service users and their carers through the promotion of Recovery focused principles and philosophy.


Advocating for service users, carers and relatives and provide appropriate support and advice to carers including referral for a carer's assessment.

Pay Enhancements - 30% additional for Evenings (8pm onwards) and Saturdays and 60% additional for Sundays and Bank holidays.
