The Role and Department:

We have an opportunity for a Navigator to join the organisation on a (fixed term) basis. Reporting to the (Project Manager), the (Navigator) will be responsible for

Supporting clients with their mental health needs by carrying out initial discussions and providing a range of guided self help services
Working with clients who present in crisis, including supporting with deescalation of crisis & risk management as well as clients with cooccurring conditions
Working in partnership with other services, specifically for this role you work alongside Adult Social Care
Working with clients who present in crisis, duties include deescalation of crisis & risk management
Navigating clients within their own communities to live well
To provide support for other teams when required)
