Band 4

Contract - Permanent. Full-Time 37.5 hours per week


The Coordinator will deliver high level quality care and evidence-based intervention, as a member of the Virtual Ward/Rapid Response team.

They will support the nurses in charge and triage team and be able to perform a wide range of administrative and clerical duties autonomously in close liaison with community matrons.

They will provide a wide range of practical care duties and be able to take appropriate action to achieve planned aims and objectives of individual clients within their homes, without direct supervision.

They will be personally accountable for the standards of such delegated duties.

The post holder will continue to develop and undertake a varied range of delegated tasks, will report to a registered professional, the role will continue to develop through gaining further skills, knowledge and competencies.


  • Answering calls and responding to queries from patient's, relatives, carers, clinicians and the wider MDT.
  • Accurate and timely recording of data
  • Working closely with Community Matrons and medical professionals to organise appropriate referrals and ongoing support to patient's in their own homes.
  • To identify changes in patient's conditions and recognise within the post holders capabilities, when to escalate concerns.
  • To communicate in a timely and comprehensive way with nursing colleagues and members of the Multidisciplinary team.
  • To be competent in essential technical skills
  • To give advice and support to patients/carers that reflects the care plan and provide a level of knowledge and experience to reassure the patient/carer and is within the competence of the post holder.
Medical services have been delivered on the Whittington Hospital site for over 500 years.
