CAR DRIVER ESSENTIAL
We have an exciting new opportunity in diabetes care to support Primary Care in the management of high-risk diabetes patients.
You will work alongside our experienced team of Diabetes Specialist Nurses and support the development of an integrated diabetes model of care for replication across the county.
Within this role you will be expected to travel around our community and work with a range of stakeholders to support the delivery of the programme's objectives.
This is a non-patient facing role on a 1 year, fixed-term contract (a secondment would be considered).
To support the team with diabetes care this may include undertaking foot checks and completing elements of the annual review.

The post holder will be responsible for delivery of safe and effective care and support of patients living with Diabetes in clinics and PCN's.
