An exciting opportunity has arisen for an experienced, highly motivated and forward-thinking Clinician to lead our 2 hour community response offer across Heywood, Middleton and Rochdale.


As HMR Rapid Response team lead you will be responsible for service development and quality improvement in delivering this model of care including virtual ward.


Effective communication across primary and secondary care is key, as well as advanced skills in providing community care with complex patients.


You will work as an autonomous practitioner assessing patients' needs and using evidenced based clinical skills alongside digital technology to care for patients in their own home.

You will manage team staff including appraisal, rota's, recruitment and selection and the supervision of learners.

Support and lead a team of healthcare professionals in the delivery of evidence based patient centred care, which is of a high standard.

You will be responsible for ensuring all care is delivered according to Trust policies & procedures maintaining associated records.

Utilising clinical audit, research, supervision of practice and teaching, you will monitor and improve standards of care by efficient and effective use of resources.
