Clinical Lead - Band 7

Devizes Community Team

30 Hours per week - Permanent

Devizes Community Team are seeking a Therapy clinician with community experience who has the ability to clinically and operationally lead on the urgent response procedure for the team and the management of complex patient care in the community.

You will be required to deputise for the Team Leader as well as work alongside a number of GP practices.

The post-holder will provide a high level of clinical assessment, individualised care planning and expert advice.

You will also be expected to contribute to national and locally agreed targets of reduction in unnecessary hospital admissions and the facilitation of earlier discharges from the surrounding hospitals.

You would be expected to work in patients homes and community clinical settings in Devizes and will typically work core shifts.

The team is based at Beacon House, Devizes.

The post requires you to undertake 3 funded modules at Masters Level; the post holder will be paid at Band 6 salary until they are completed and then will be moved to Band 7 salary.
