An exciting, evolving role for an Advanced Practitioner, (having completed the advanced practice diploma/Msc), who is experienced and enthusiastic about acute care choices at home, to provide a hospital at home service in patients homes/care homes, to avoid hospital admission and expedite hospital discharge.

To work as an advanced practitioner in an autonomous role to assess, diagnose, treat and evaluate care and develop themselves and the service in the pillars of advanced practice for quality patient care at home
