Description
The Reablement Team is a multidisciplinary team consisting of Occupational Therapists, Physiotherapists, Therapy Technician and Support Workers.

The Reablement Team are a community rehabilitation team delivering goal-focused, intensive therapy over a period of 6 weeks.

The team responds to hospital discharges and those needing an increased level of support in the community to avoid hospital admissions and to reduce hospital lengths of stay.


As part of the Team you will be providing active rehabilitation for people within their own homes, enabling people who no longer require hospital based care to continue to maximise their independence within the community.

The team will work with individuals to regain the skills they need to maintain independent living within their own homes, including:


  • Moving / Walking
  • Personal Care
  • Kitchen Skills
  • Memory / planning
  • Community support
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Supporting the Clinical Occupational Therapists (OT) and Physiotherapists (PT) in the delivery of OT and PT management and rehabilitation to community patients/clients within the Local Authority (LA)


To work as part of a community based integrated health and Local authority intermediate care and rehabilitation team to undertake specific clinical and administrative tasks, implement intervention programmes within the service user's home and community as instructed and delegated by the OT and PT, responding flexibly to the needs of the service.


Support the PT and OT staff by assisting with all modalities of therapeutic care for intermediate care and reablement patients.


To provide care to the patients, as delegated by the PT's and OT's DT, to the extent of own competencies.


Participate in project work, audits and provision of relevant statistics which may be for the benefit of patients or the PT/OT and Local Authority department.

To promote independence, health and well-being and safety in hospital, home or community settings.


Develop a patient centred approach to engaging patients in routine therapeutic occupations following the aims/methods given by the Therapist and to adjust own clinical interventions based on the fluctuating needs of the person.
