Rehabilitation Support Worker, Merton Discharge to Assess - Band - Merton.
We are seeking an enthusiastic Band 3 Rehabilitation Support Worker to join our Community Discharge to Assess service working alongside Physiotherapists and Occupational Therapists.
Our goal is to optimise our patients' independence and wellbeing, enabling them to return to their own homes following a rapid decline through illness or injury, including falls.
Key responsibilities include:
- To perform therapy and nursing procedures where necessary for which adequate training has been given.
- To work under the supervision of the professional staff in the multiagency team in the rehabilitation of patients/clients.
- To be conversant with the care plans of those patients/clients with whom you work directly, prior to each intervention.
- To work to agreed standards of care within the D2A team and always to consider the needs and wishes of the patient/client.
- To report any change in circumstances to the appropriate professional for whom you are carrying out a delegated task.
Successful post holders must be able to work 37.5 hours across 7 days per week and at several locations where the services require it.
The service operates between 8am and 8pm.A commitment to personal and service development is essential.

To work under the supervision of the professional staff in the multi-disciplinary team for the care and rehabilitation of patients.
To contribute to the on-going assessment of a patient by completing the appropriate paperwork, recording and reporting all clinical and behavioural information to the nurse/therapist for action and planning purposes.
To be conversant with care plans of those patients with whom you work with directly prior to each intervention.
To follow rehabilitation plans independently without supervision, to review, update and report back within defined delegated responsibility framework.
