Description

This is a really exciting opportunity to contribute to our MDT for people with a learning disability. In the role you will work closely with Nurses, Psychologists, Physiotherapists, Occupational and Creative Therapists. You will also collaborate with family carers,Social Workers, Commissioners, the NHS and providers of residential care and supported living.

Reporting to the Clinical Health Team Manager you will be required to deliver the best outcomes for the health of adults with a learning disability.

This role supports the commitments set out in Suttons Health and Social Care LD Strategy to help improve access to work, health, independence and social participation.

You will work collaboratively deploying your specialism to support the work of the wider team.
