Band 6 IDT nursing sister.

Based within The Integrated Discharge Team - Lister Hospital Stevenage.


An exciting permanent opportunity had arisen for enthusiastic clinician to work within the Integrated Discharge team based at Lister Hospital as a Discharge Planning Nurse.

You will be responsible for planning complex discharges from Lister Hospital in to the community.

You will be involved in assessing for Continuing Health Care funding.


You will work within the multi-disciplinary teams to develop robust discharge plans for complex patients to ensure their discharges are safe and timely.

You will have excellent communication and negotiation skills in order to liaise with staff, patients and relatives.


The post holder will have responsibilities to follow up patients and assess patients in the community and their own homes.
