Responsible To:
Service Manager


Summary of Post:
To support the Service Manager, the people we support and the teams to design, deliver, maintain and develop exceptional person led support packages


Job Description:


  • To be first point of management contact: Visiting services, listening, developing relationships with all staff and supporting Senior Practitioner's/ Team Leaders
  • Carry out agreed auditing and quality checks: Ensure all paperwork is being completed correctly to the best standards
  • Complete Service User Risk Assessment: Supporting the team to risk assess the service user's home, health concerns, activities and support worker safety whilst performing care making sure safety is paramount for all parties ensuring policy and procedures are in place to safeguard the people we support and their staff
  • Help facilitate team meetings: To review support that's being provided looking at progress towards outcomes and making plans for new outcomes.
  • Conduct supervisions: With support workers to go through progress, supporting them to reflect on their practice, identifying training requirements, giving staff and service user feedback Along with feedback on personal development. To report back in a timely manner to the service manager highlighting any issues/actions.
  • Support Worker development: To conduct spot observational practise on support worker performance whilst working. Drafting up action plans with the worker if needed for any additional support or training required.

Service Manager:
Feedback to Service Manager on a weekly basis on compliance actioned, compliance outstanding, no of visits, complaints and compliments recorded

  • Attend client/social work reviews: Deputising for Service Manager and feeding back, ensuring actions are put in place when required
  • Attend discharge planning meetings: Meet with multidisciplinary teams to discuss plan for discharge from hospital/care setting. Working with the rota coordinator to put a care package together that benefits the service user
Essential

  • Experience of working in a care setting minimum 2 years'
  • Experience of writing support plans/ risk assessments
  • Exceptional customer service delivery (relationship building)
  • Excellent time keeping and diary management
  • Strong communication skills, both written and oral
  • Good Understanding of Care Standards.
  • IT literate
  • Team player
  • SVQ MIN 3
Desirable

  • Good understanding of Mental Health and Learning Disabilities
  • Driver with access to a car
Key Competencies

  • To be able to establish and maintain effective relationships with key stakeholders.
  • To be able to prepare and review robust care plans and report actions to Service Manager.
  • To be able to maintain all administration records that are your responsible for up to date.
