Description

This exciting role of the Complex Care Lead is responsible for supporting the development of aspects of the Trust's clinical strategy relating to Frequent Caller management and the receipt of patient information to support response and treatment.


The role will largely be based from home with occasional need to meet with both internal and external partners, across the South West.


With support from the Head of Safeguarding and the wider Quality & Clinical Care directorate, the Complex Care Lead is responsible for ensuring appropriate external relationships are maintained to result in safe, effective, well governed and appropriate processes are in place to support complex cases, including high frequency users and complex clinical care.


The post holder will work collaboratively with key partner agencies, nationally, regionally and locally, agreeing and delivering services and/or procedures that meet the varied requirements of people with complex care needs, including the provision of data to support creative models of care management for frequent callers and patients with complex care needs.
