An exciting opportunity has arisen for a Senior Discharge Co-Ordinator to join our team within the Community Hospitals across Dorset HealthCare University NHS Foundation Trust.

This is a new post.


The post holder will work within a fast paced and dedicated team who are passionate about supporting patients discharges and facilitating flow through the community hospitals.


The post holder will work alongside the Discharge and Flow Matron to be a visible support for the discharge co-ordinators in all 8 community hospitals in Dorset.

There will be opportunities for further learning and development within this role.


The primary purpose of the post holder is to provide a visible presence across the 8 Dorset Community Hospitals to provide supervision, training, and support for the discharge co-ordinators.


To ensure a consistently safe, responsive and patient-focused service provided by the discharge co-ordinators across DorsetHealthcare Community Hospitals, 7 days per week.

To escalate issues as appropriate to the Discharge and Flow Matron

To represent and uphold the values of the Trust at all times.

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check.

Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.
