About the Role:


Central Basildon PCN has an opportunity for a care co-ordinator to join our primary care network (PCN), to support our 9 GP practices across the Basildon area to assist in re-shaping primary care across our communities.


The role involves working very closely with the practices and the multidisciplinary team (MDT) within the PCN, and is pivotal in ensuring that all patients receive the best possible care and service.


The main duties of the role include:

  • Working closely with stakeholders on Integrated Neighbourhood Team projects;
  • Speaking to patients directly and coordinating their care;
  • Supporting the clinical directors in coordinating all key activity, including access to services, advice, and information, as well as ensuring that health and care planning is timely, efficient, and patientcentred;
  • Supporting digital initiatives;
  • Responsibility for the coordination of the patients' journeys through primary care.


If you are a skilled care co-ordinator, with excellent person-centred planning experience and strong local knowledge for accessing key support, then we would love to hear from you.

The ideal applicant will have have the following skills, experience and qualifications:

  • Experience of working within healthcare, the voluntary or community sector, to support vulnerable groups;
  • Ideally you will have a Diploma Level 2 in Health and Social Care, or an equivalent qualification;
  • Strong listening and communication skills;
  • Familiar with local resources and services, including how to access them;
  • Motivated by helping people, with care and empathy;
  • Able to deliver personcentred support in a nonjudgmental way; Experience of completing holistic personcentred care planning assessments.
