Description

The post holder is responsible for leading the Neonatal Community Outreach Team, the assessment of care needs, the development, implementation and evaluation of programmes of care without supervision in both the hospital and the community setting.


The post holder will work across site in both hospitals and Community environments when required. The post holder is required to teach and supervise families and other nursing and non-nursing staff and be a resource person within both units for discharge planning and outreach support. The post holder is required to have flexible working hours. Taking continuing responsibility and accountability for the management and organisation of the community team. This involves managing a wide range of complex and sometimes conflicting priorities**
Assess needs and facilitate high quality, will work in partnership with the parents, empowering them to meet their baby's on going special needs following discharge in collaboration with the multi professional team, and in line with the local and national guidelines and policies.
**At all times ensure that the needs of the baby are placed at the centre of care delivery, monitoring standards and taking action to maintain and improve them. Manage a team of staff ensuring appraisals and appropriate professional development takes place. Identify and manage performance issues in line with Trust policies. Participate in recruitment and selection and the clinical governance agenda
