An exciting opportunity has arisen to work as an administration staff / health care assistant (HCA) in a Community Respiratory Service in Hounslow, as part of Integrated Care service line at West London NHS Trust.
This is an excellent opportunity for an enthusiastic and committed person to join new Community Respiratory Service in Hounslow, as part of Integrated Care service line at West London NHS Trust as a Band 3 Administrative / HCA.
Hounslow Community Respiratory Service, support chronic respiratory patients in the community with managing their conditions, providing them with support to Self-Manage.
- Helping with the daytoday running of the team as a whole which may in addition include organisation of appointments, liaising with GP Practices for patient information, triaging and prioritising telephone calls from patients and the wider MDT
- There will also be some general administration duties including uploading of clinical information, photocopying and supporting the Physiotherapist / CNS with ad hoc activities

We are seeking a nursing and physiotherapy profession to provide direct care for patients with Respiratory conditions the community based Oxygen and Respiratory Case Management Team, including Pulmonary Rehabilitation for Hounslow residents using a multi-disciplinary team approach.
The service is to provide:
- Home Oxygen Service
- Assessment and Review (HOS-AR)
- Case Management and support of patients with unstable or more complex respiratory disease.
- Primary Care Education and Support.
- Pulmonary Rehabilitation service; Supporting patients to manage their breathlessness through exercises and improve their functional abilities.
