Description

This position will be to support patients within East Riding Primary Care Networks to have their annual Health Check for Severe Mental Illness completed.

The check will involve completed checks such as blood pressure, BMI, cholesterol and blood glucose alongside discussions around current lifestyle, healthy living and signposting to relevant services.

You will manage and prioritise a caseload of patients to support the completion of their health check.

Alongside this you will identify when a person's needs are beyond the scope of your role and will be required to work with the GP and signposting or referring for ongoing support elsewhere.


The main duties of the role will include:

  • To complete the annual Health Check for Severe Mental Illness
  • To provide 11 support for people based on what's important to them
  • To manage and prioritise a caseload, in accordance with the needs and support required by individuals. Along with a strong awareness and understanding of when it's appropriate or necessary to refer people back to other health professionals/agencies, because the person's needs are beyond the scope of the role
  • To work as part of a multidisciplinary, multiagency team to promote health coaching to support Personalised Care and Supported Self-Management, modelling the coaching approach in their work.
  • To attend and contribute to team meetings and events as required
  • To work flexibly, adapting to the needs of the service and client group while maintaining the integrity of the role
  • To participate in regular health coaching supervision and continual learning including:
  • Regular contact with supervisor
  • Refresher training sessions
  • Buddying with peers
  • Peer support sessions
  • 1:1 support from a practitioner with more health coaching experience
  • Action learning sets
- e-learning to revisit or deepen training

  • Ongoing improvements to systems and processes
  • Collect service user experience and impact of health coaching as part of the delivery of personalised care
