This role is an integral part of the Integrated Discharge Service supporting patients across the Pennine Lancashire footprint.

The role will work with patients, families and clinical teams to support their care needs and associated care packages and placements either on discharge from acute care or community placement or to support a community crisis.

The role will work with the discharge to assess model of care/ continuing health care/ end of life/ mental health/specialist care and home first delivery team.

The role will operate using a trusted assessment and work to support the work of case managers in providing and supportive and highly responsive service.

The service will operate 7 days per week including bank holiday and hours will be flexible 8am - 8pm.

  • Support the onward care and planning of patients on pathways to provide a proactive and efficient service from admission to discharge, ensuring all patients receive safe, personal and effective care throughout their stay.
  • Sourcing short/long term care packages/placement for a whole range of personal needs and specialist needs.
  • Work in partnership with other health and social teams and services across the hospital(s) that are involved in discharge planning or transfer of care for patients to alternative settings.
  • The post holder will be required to maintain a confidential and professional approach at all times. The role will involve communicating with patients, relatives, colleagues, nursing and AHP professionals across health and social care, either by telephone or face to face.

  • Work flexibly across all Intermediate Care Services sites to meet the needs of all discharges and transfers of care for all adult patients being discharged or requiring community support.
  • Drive communication and response to maintenance of patient flow in support of organisational targets.
  • To ensure you are an integral part of a responsive team are delivering a highly responsive and effective service delivery.
  • Complete a daily work allocation and ensure completion of all tasks to effectively operate the service, within this demonstrate a flexible approach to meet individual patients/family need.
  • Report & monitor problems with patient transport services and escalate issues to senior clinical flow team.
  • Provide proactive contribution to allocated wards to ensure length of hospital stay is determined by clinical need only and not by organisational resources.
  • To promote and maintain effective communication channels between patients, their relatives and relevant teams in order to successfully coordinate discharges.
  • Proactively advise and support staff to initiate the early discharge planning of patients with simple needs ensuring appropriate assessments have been completed in a timely manner to facilitate discharge.
  • Identify, progress chase and monitor delayed discharges and lost bed days within the patient journey and referrals.
  • Screen and input information of onward referrals made to the homes and health and social teams, including the completion of trusted assessment documents.
  • Maintain accurate records and participate in audits as required in relation to the discharge planning process.
  • Support and respond to the Trust Escalation Processes, as guided by the IDS Business Manager, Head of Site Operations & Flow / Head CCM/ Senior Directors / Managers.
  • Ensure compliance with other related Trust Policies and Department of Health Legislation with regard to discharge planning processes.
