We are looking for an energetic and innovative AHP Health care support worker to join our Community Rehabilitation teams.

The post holder will work in the community across the Home First and Community Hospital therapy teams.
This post holder will be working primarily with people with general frailty, who have fallen or at risk of falling.
As part of the therapy team the post holder will be allocated a delegated caseload by the qualified AHP and be expected to work independently, assessing the patient's care needs and/or modifying existing treatment plans and delivering this care without supervision.
This includes assessing, progressing, reassessing and discharging people within the scope of practice. You will have excellent communication and interpersonal skills to complete clinical and non-clinical duties together with service development activities.
Ongoing personal and professional development is essential to provide a quality service and is supported through CPD, appraisals and in-service training.
