The Doncaster Assertive Outreach Team is looking for a registered mental health nurse (RMN) to join the team, offering exceptional support for individuals with an enduring psychotic illness and complex needs within the local area.

This post is an exciting opportunity for a registered mental health nurse with significant post grad experience who is looking to develop their knowledge and skills by working within a community-based setting.


We believe our services to be a hard working but friendly, supportive, and nurturing environment, with many established and experienced practitioners.

Our case load size is smaller than other community teams. Which enables us to offer intense care and quality time to people who have rehabilitation and complex needs.


We work within a multi-disciplinary team with other community services to support and aid individuals to promote their independence and social inclusion within the community.

Striving to achieve positive outcomes on their recovery pathway.

The care co-ordinating role is one of great importance within the recovery pathway. It allows individuals and their families to experience collaborative work between primary and secondary NHS services and different agencies.

The main roles of a care co-ordinator are to be able to assess, formulate, implement, and evaluate the care and treatment plans to a high-quality standard.

Care coordinators will also need to be able to assess and manage risk within a community setting with the support of group and individual clinical supervision.

To liaise with other services and members of the MDT to see what holistic care package can be offered to the individual to aid their care within the community.

To fulfil this role, you will need to be able to travel independently around the Doncaster area.


We are open 8am-8pm Monday to Friday and 9-5 on a weekend, we have flexible working options available, and all staff have agile working equipment.
