To play a pivotal role as an extended member of the Colorectal Cancer Multidisciplinary Team, working closely with nursing and medical colleagues, in supporting the administrative aspects of the service.

Providing support to the specialist nursing service in order to maintain effective clinical management of patients with Colorectal cancer.

The role will involve and being responsible for aspects of direct care delivery, assessment of patient and carers needs and the provision of general information, help and support; ensuring that patients and carers receive holistic, multi-faceted support, co-ordinated care, and all the information they need, throughout the course of their illness.

And ensuring effective communication across the Trust, Primary Care and with statuary and voluntary agencies in relation to patient information.

The overriding purpose is to support the provision of the highest quality patient care through personal actions and continuous improvement.


  • To work as a member of the Colorectal cancer team, providing administrative support and coordination of the provision of high quality patient care for people affected by Colorectal cancer.
  • To contribute to the multidisciplinary discussion as an extended member of the MDT, supporting the Colorectal Cancer Specialist Nurse in the assessment, care planning and coordination of care.
  • Demonstrate the ability to recognise indicators of patient need, or change in condition, and escalate complex problems or enquiries to the CNS or other appropriate healthcare professional or department as appropriate.
  • To provide general information and support regarding cancer and cancer services, to enable patients to navigate the health and social care system, and make optimal choices. A key aspect of the role involves daily direct and indirect communication (e.g. telephone and face to face) with patients, relatives/carers, and other health care professionals.
  • Coach patients and carers on how to make contact when they feel that their condition or needs have changed, including what to do out of hours.
  • Demonstrate the ability to recognise and respond appropriately when faced with a sudden deterioration, urgent or specialist need, alerting the team or enabling rapid reentry into the system as appropriate.
  • To support the multiprofessional team in enabling patients to be discharged and able to remain in their preferred place of care by responding to their needs in a proactive way.
