Are you motivated to deliver excellent patient care and have the experience to lead others to enhance their own care delivery? Then we'd like you to come and work in Enhanced Bed Management (EBM) as a Discharge Facilitator.


We are a close-knit team of experienced and highly motivated individuals who strive to ensure CNTWs patient population can access the care they need at the right time, in the right place and from the right services.


By joining EBM you will play a pivotal role in the care of those in CNTWs inpatient wards, working collaboratively with urgent, planned and inpatient care services to ensure safe, high quality care.


You will bring to the role your own clinical excellence and excellent communication skills however you will be supported to develop the skills and knowledge needed to support patient flow through inpatient services, in the face of persistent bed pressures.

The work involved is fast paced, dynamic and challenging however brings with it great job satisfaction.

Enhanced Bed Management are a trust wide service responsible for coordinating flow through CNTW's inpatient wards.


We are a hard-working and passionate team who strive to succeed in our role and work collaboratively to support colleagues and other service to achieve this goal.


  • Incentive payments will not be given to applicants who have previously held a substantive role within inpatient services or a bank worker post within the trust within the past 2 years.


Undertake collaborative actions with inpatient MDT, Community Mental Health Services, Statutory care providers, Third Sector care providers, Service Users and Carers to facilitate safe and timely discharge of Service Users from hospital.

Provide specialist clinical advice to Service Users/Carers, staff, statutory agencies, 3rd sector partner agencies and the public.


Work into designated inpatient areas to facilitate effective interventions, service user flow and safe, timely discharge for all Service Users in those inpatient areas.


In collaboration with Inpatient MDT, Care Coordinators, CRHTs, Service User and Carers where appropriate, develop Recovery and Wellbeing focused Care Plans for implementation by CRHT, ensuring regular reviews to monitor progress and adapt Care Plan as necessary.

Provide community interventions to support discharge plans including home visits, 72hr follow up visits as required.


To consider Carer needs and ensure Carer and family are involved in Care Planning where appropriate to deliver evidenced based Clinical and Psychologically based Interventions.


The post holder will work as a member of the Multi-disciplinary Team ensuring that high quality individualised care is delivered which ensures safety and a commitment to promoting recovery and wellbeing and maximising independence.

Provide clinical supervision to staff as delegated by Service Manager.
