We are seeking a Pathway Co-Ordinator to work across Liverpool and Sefton Complex Needs teams within the Community & Mental Health Division.
The purpose of this post is to provide a comprehensive administrative service, main duties will include planning, organizing, directing and coordinating the work of the service.
Following a successful bid, the North Mersey Region has been selected as a pilot site to implement the Integrated Care Framework (Community) in England which includes the establishment of Complex Needs Teams in Liverpool and Sefton.
The Framework has been developed as a response to the NHS England & NHS Improvement Long Term Plan commitment to provide additional support for the most vulnerable children and young people with complex needs across multiple domains between the ages of 0-18.
