Description

To work as a practitioner within a multi-disciplinary team, supporting therapeutic intervention in order to promote independence and expedite discharge home.


  • To work across Front Door services (ED, AMU, FES and shortstay).
  • To participate in 7 day working within the Therapy Service.
  • To carry out assessments as allocated by the coordinating therapist and to work without constant direct supervision within different settings including ward areas.
  • To provide therapy specific interventions to meet clients treatment needs.
  • To provide technical support and advice to other members of the multidisciplinary team.
  • To manage own caseload on the ward (with support from the qualified therapist as appropriate)
  • To construct, implement, monitor, evaluate and update therapy careplans / treatment programmes
  • To support ward staff with careplans / treatment programmes promoting ongoing rehabilitation.
  • To work independently and as a lone worker in a range of settings including ward settings.
  • To independently carry out and record assessments of an individual's function. Liaising with supervising therapists and other MDT members when appropriate.
  • Provide relevant intervention using a combination of techniques, health promotion, equipment and adaptations to maintain, restore and promote independence in the community, taking into account social, cultural and economic needs.
  • To carry out and deliver care plans and treat patients individually or in a group in a variety of settings without the presence of a therapist.
  • To effectively communicate with clients, family / carer who may have barriers to understanding or communicating e.g. blind, deaf, or who do not have English as a first language.
  • To undertake appropriate communication with members of the multidisciplinary team, outside agencies and others i.e. clients, carers, relatives etc, community based colleague.


Walsall Healthcare NHS Trust is an integrated Trust and the only provider of NHS acute care in Walsall, serving a population of 286,700, providing inpatient and outpatient services at the Manor Hospital as well as a wide range of services in the community.

Walsall Manor Hospital houses the full range of district general hospital services under one roof.

The £170 million development of our Pleck Road site was completed in 2010 and the continued upgrading of existing areas ensures the Trust now has a state-of-the-art Critical Care Unit, Neonatal Unit, Obstetric Theatre, and Integrated Assessment Unit facilities.

Work is also well underway on its multi-million pound new Urgent and Emergency Care Centre which will house a new Emergency Department (including Children's Emergency Department), co-located Paediatric Assessment Unit, Acute Medical Unit and Urgent Care Centre.

This is scheduled for completion this year.


We provide high quality, friendly and effective community health services from some 60 sites including Health Centres and GP surgeries.

Covering Walsall and beyond, our multidisciplinary services include rapid response in the community and home-based care, so that those with long-term conditions and the frail and elderly, can remain in their own homes to be cared for wherever appropriate.


  • To feedback appropriately and timely to the multidisciplinary team.
  • To attend multidisciplinary team meeting and any other appropriate meetings as the therapy representative as delegated by the therapist. By use of feedback contribute to the clinical decision making process of the MDT.
  • To record activity on a daily basis in medical notes and on any other systems required.
  • To liaise with the therapist regarding patient care in particular highlighting any variations to expected outcomes from treatment programmes delivered at the earliest opportunity.
  • To attend and participate in access visits with or without a therapist. To record outcomes of environmental assessments and submit reports where appropriate.
  • To advise, use demonstrations and teach appropriate job skills to new or less experienced staff as part of the induction process of new staff and students.
  • To adopt appropriate postures in treatment of patients, complying with moving and handling and risk assessment policies of the Trust.
  • To maintain records, electronic and paper in a timely manner and in line with organisational requirements.
  • To coordinate and prioritise the daytoday activities of own caseload.
