Are you a Support, Time and Recovery (STR) worker with a passion for delivering outstanding care that brings the 6 Cs (care, compassion, commitment, courage, communication and competence) to life? If so, we want to hear from you.


We are looking for an experienced and highly committed STR worker for our Homeless outreach team service to support us in ensuring we deliver outstanding patient care.

The service provides initial assessment of mental health needs, alongside intensive home treatment intervention to help promote community health and well-being and social inclusion.

The team operate Monday to Friday 9am til 5pm and provides support to service users homeless in the local community

Shortlisting date is planned for 27th February 2023

Interview date is planned for 6th March 2023


The post holder will work to assist with the continuity of care for service users within the appropriate care setting.

The post holder will work from the community environment.

Under the direction of the Qualified Practitioner, participate in the delivery of planned care within both in Service User and community environments.

The post holder will undertake a range of duties which include:

  • Facilitate and support the development of appropriate personal care
  • Facilitate and support self help, communication and social skills acquisition
  • Assist and promote independence with activities of daily living
  • Facilitate a service users recovery by assisting them to participate in social, vocational, recreational and leisure activities within the appropriate setting
  • Promote social inclusion through developing opportunities in areas such as employment, volunteering and education.
  • Participate in the implementation of specific programmes of care or therapy to develop and enhance socially appropriate adaptive behaviours under the supervision of qualified staff
  • Participate in regular supervision, engage in reflective practice and be committed to continued development
  • Provide information to enable the effectiveness of care to be evaluated.
  • Ensure appropriate records are maintained under the direction of the qualified practitioner.
  • Contribute to the involvement of Service Users, and carers/relatives in the delivery of care
  • Provide reassurance and support to service users and carers/family
  • Work collaboratively within the Care team to ensure that Service Users needs are met
  • Recognise and respond appropriately to urgent and emergency situations
  • Contribute to the development of services
  • Contribute to the effective and efficient use of resources
  • Help to identify early signs of relapse as described within the care plan and alert the appropriate staff involved in the service users care.
  • Understand and adhere to Trust policies, procedures and clinical guidelines
  • Report any concern regarding Service User care to the Qualified Practitioner
  • Contribute to service improvement initiatives to enhance quality of Service User care
  • Participate in Service User and public involvement activities
  • Support individuals equality, diversity and rights
  • Participate in Serious Untoward Incident investigations/reviews as required
