Remedy Recruitment Group are currently on the lookout for a Senior Social Worker for an Adults Team based in South West England.

The aim is to support the individual within their own home. Attending MDTs to confirm when Building Independence Team assessment will be most appropriate.

Taking a strength-based approach when completing Care Act assessments and Support Plan (if relevant)for individuals that are receiving reablement, pre-assessment or D2A support.

Arrange and coordinate ongoing support (post-assessment) for the individual, including Carers Assessments if relevant Complete Service Set Up Review of post-assessment support/serviceideally within 4-6 weeks Ensuring a comprehensive handover to the next allocated team (if applicable) and all ongoing referrals


You will need to be:


  • SWE registered
  • Eligible to work in the UK
