Description

Our local authority client in York are seeking full time and part time Social Workers to join their Hospital Social Work Team.

The full-time role is Monday to Friday and the part time role is 2.5 days, Monday to Wednesday or Wednesday to Friday to start ASAP.

This is for cover until 31st March 2023 (covering secondment) and work will be mostly based in offices located close to YorkHospital, though some home-working may be feasible.

Core responsibilities, tasks and duties are:

Rapid Assessment under Discharge to Assess guidelines for hospital discharge.
Assessing service users, mental capacity and making best interest decisions.
Liaising with the discharge liaison team, as well as ward staff and family members.
Arranging care packages on discharge and step down beds.
Completing reviews and support plans.
Reporting any delays with service users.
A good understanding of hospital social work.
A minimum of 2 years recent Social Work experience within an Adult Social Work team.
Good knowledge of the Mental Capacity Act and the Care Act
A relevant Social Work qualification (BA/MA Social Work/DipSW or equivalent).
IT skills, preferably experienced with Mosaic.
SWE registration
Be eligible to work within the UK.

Pay will depend on qualifications and experience.
