The Associate Practitioner will work closely and in consultation with Community Discharge Service Multi-Disciplinary team: Occupational Therapists, Physiotherapists and Social Workers.

They will be involved in delivering interventions that can optimise patient flow through community services, and to improve outcomes / experience for clients once discharged from hospital.

Requirements:


  • NVQ 3 or equivalent in a health related topic
  • 2 years relevant experience in an healthcare administration setting
  • Full UK driving license and access to a car
  • Right to Work in the UK
