Job Description_

This is a new post caring for young people in a Homecare Settings.
Keyworkers will provide support, including practical and emotional support to young people with complex health care needs. The role is to develop individualised package of care and support of a young persons.

Keyworkers will also offer advocacy to ensure young persons are heard whatever their communication needs and will identify and address cross organisational challenges.

Keyworkers will work closely with all agencies involved with the young persons across health, social care and voluntary sector. Working with multi-disciplinary teams so being able to have knowledge of local support agencies.

Principals Accountabilities_


To work with young people who have complex needs, and other agencies across a range of services, e.g., social workers, to assess their support needs and draw up, implement and review support plans in line with the aims of the services.


To work alongside with young people and their social workers at BenArisHomeUk, offering practical and emotional support and creative responses to their needs.

To act as a positive role model, establishing guidelines for Behaviour, giving social, spiritual and emotional support.

To create a safe and happy environment in BenArisHomeUk to live.

To give advice and support independent living to those who are about to leave care.

Keyworkers will be responsible for supporting a young person Ratio 1-2 depending on each individual young person's care plan.


To work closely in partnership with a range of multi-agency partners including GP's, Dentist, social workers, Educational, Job Employments agencies, Mental health team, Therapists, leisure services and assists service users to access, build and maintain effective relationship with them.


To keep an excellent service users' record, write reports such as daily records and weekly records and ensure information is clearly written, always maintained and up to date.


To be able to participate and contribute to planning, strategy, case conference meetings with external agencies and to involve service users in case planning meetings.


To ensure you have an understanding of your role as a keyworker and comply with BenArisHomeUk procedure for promoting and safeguarding the welfare of vulnerable young people.

To work Flexible including long days, nights, weekends, Sleep ins as may be required by the needs of the service and carry out any other reasonable duties.

Person Specification_


  • An NVQ Level 3 in social care or other relevant qualification is required.
  • Experience of working with Children and young person with complex health care needs.
  • An understanding of working with young people.
  • An understanding and professional manner, with an ability to use initiative creatively when working independently with young people.
  • Experience of multi-agency working.
  • An ability to work collaboratively with young people, to identify their support needs, plan, implement and monitor emotional and practical support to a young person.
  • An understanding of the impact of discrimination and an ability to work with people from diverse backgrounds.
  • Good verbal communication skills and the ability to keep good written records and to give accurate information.
  • An ability to work independently and as part of a team within a framework of policies and procedure.
  • An ability to receive training, advice and constructive feedback in regular supervision and appraisals.
  • An ability to work flexible, long days, nights, weekends, and sleep ins.

Salary:
£9.80 per hour


Schedule:
