Advanced Clinical Practitioner - Band 8a

Virtual Wards - All Wiltshire Localities

37.5 hours per week, Permanent


As an ACP you will be based within an Integrated Community Team and be part of the services which wrap around our patients, putting them at the centre of their care.


The post holder will be expected to work multi professionally, support other team members roles and contribute to the future development of Community Services.
