Job Purpose:
To assist in the data quality improvement process for Clinical Services patient activity data.

To identify, highlight and action duplicate patient registrations, incomplete demographic data items, invalidated NHS Numbers and to assist in the management of waiting lists for reporting purposes Duties and Responsibilities:
The post holder must have a sound understanding of patient activity data.

Data must be accurately and consistently recorded to well defined standards to enable it to be used for statistical analysis, both locally and nationally.

The post holder will have a working knowledge of Data Protection and Data Security requirements.


The post holder will explore and remedy unusual or missing demographic data items and liaise with users across the Trust over a variety of data quality issues, explaining errors, discussing impact and ensuring corrections are made.

Monitor and update patient demographic details and input missing NHS Numbers on all locally registered patient records by using multiple source systems.

This is necessary to obtain a single unique instance of each patient for use in the NHSCR (electronic patient record).

The post holder will deal sensitively with invalidated patient records possibly due to adoption, gender reassignment, secure identity etc and ensure that these records have not been contaminated with incorrect data.


Regularly investigate patient records that could be duplicates following procedures to ensure they can be merged without loss of any data.

Where records cannot be merged, ensure they have an appropriate identifier to enable them to be identified at a later date.

The post holder will work within procedural guidelines and to established precedents, generally without supervision, although support is available for reference.

They should make a judgement on when it is necessary to involve their line manager on issues requiring more senior attention.


The post holder will review, prioritise and plan their workload on a regular basis within the workload delegated from the Information and Data Improvement Manager.

The post holder will regularly analyse a given situation where an issue has been identified and use their initiative in making a considered decision on the best course of action.

The post holder may come into contact with patient details that maybe distressing or emotional in content including the management of data linked to deaths or adoptions/safeguarding sections.

Play an important role in assisting with the management of waiting lists across clinical services to ensure accuracy and validity for reporting purposes.


Provide specialist advice and guidance with regard to data definitions, standards and proper recording of demographics and community activity information.

Have an input in the creation and running of Data Quality dashboards within Qlik, extract data from the Data Quality reports and deliver outcomes, findings and recommendations to clinical services.

Communicate fairly complex technical aspects and idiosyncrasies of SystmOne (S1) functionality in a non-technical way to clinical services staff and clinicians whose use of clinical systems contributes to waiting lists management.

Provide support, guidance and training for staff The nature of the post demands that the post holder multi-task and maintain concentration levels, despite the work-pattern occasionally being unpredictable.


There is significant VDU work associated with this role requiring dexterity of both mind and keyboard to regularly manipulate complex data at speed.

Provide weekly/monthly progress reports to the relevant clinical services. Use various software programmes to produce reports, statistical data, create and maintain spreadsheets to a high standard.

Job Accountabilities Resolve issues efficiently to assist in the timely and accurate capture of data for use in financial, other corporate and clinical initiative performance reporting.


Support the Information Team and the Information and Data Improvement Manager in the production and auditing of patient pathways that ensure achievement of the waiting time targets, assist in the maintenance of waiting lists and provide housekeeping and advice to ensure complete and accurate timely content.

Monitor and support agreed data inputting standards with the use of data manipulation techniques developed by the Information Team. The use and development of procedures and guidelines is vital for ensuring consistency in recording methods from service users. Test and comment on policies, procedures and development documentation.

Create, manage and share standard operating procedures (SOPs).

Report regularly to the Information and Data Improvement Manager and attend regular update meetings with the rest of the team when required.

Support the Information and Data Improvement Manager with departmental reviews of work and policies and in the delivery of effective team meetings with clinical services.
