Description

The post holder will be required to work independently, but under the guidance and supervision of the Service Line Management Team and Skin Cancer Nurse Specialist Team to set up this exciting new role to navigate patients through the new Skin Faster diagnostic Cancer Pathway.


They will be based primarily within the Plastic Surgery and Dermatology Department and will act as a point of contact for patients who have been referred via their GP with a high risk of having a Skin cancer.


The Navigator will also be required to collect data and prepare statistical reports to track patient activity through the new pathway.


Preference will be given to internal Trust staff, as well as 'Priority' and 'At Risk' status including NHS At Risk staff throughout Devon who are able to display recent relevant experience as dictated by the Person Specification.

Key Dimensions:


To navigate and support patients through the Skin cancer pathway using own judgement and initiative utilising specific departmental and patient administration systems ensuring strict adherence and compliance to all Trust Policies and procedures.


To co-ordinate appropriate investigations required, signpost patients to appropriate support and assist with 'Making every Contact Count' to ensure the delivery of patient care is of the highest standard and those patients and their families are treated with dignity and respect.

To utilise iPM/iSOFT and other systems/databases to track patient and upload relevant cancer data.

To set up and maintain new databases of patients coming through the new pathway. To analyse data and to prepare reports for the Clinical teams to help develop the Skin cancer service.

Ensure they are familiar with the Risk Management Framework.

Coordinate care for patients coming through the new Faster diagnostic standard (FDS) Skin Cancer Pathway

Work with a range of clinical and non-clinical staff in the Skin cancer pathway.

Provide and receive complex information about patient care and interpret the patient plan for other clinical and non-clinical staff.


A key aspect of this role involves daily and direct (eg face to face and telephone) communication with patients, relatives and carers and other health professionals.

The information and nature of communication required is sensitive due to the nature of cancer and therefore communication in this context requires a high degree of empathy, understanding, diplomacy, honesty and integrity.


To have a good knowledge and understanding of the new National FDS Cancer Pathway and an understanding of the radiology tests arranged, and further tests that may subsequently be requested.


To provide non-clinical advice and information using good communication skills to navigate patients through what will be a time of great anxiety and where patients may be highly distressed.

Following up and tracking results and acting on results in a timely, appropriate manner.

Support the delivery of patient education using 'Making every Contact Count' with specific focus on smoking/alcohol cessation.

Provide basic advice and refer on or signpost to other sources of support.


Co-ordinate and organise appointments and assessments as required and to exchange that information clearly and concisely dealing with anxiety, cultural or language difficulties.

Plan and organise complex programme for co-ordination of patient pathway.


Co-ordinate the handover with other Teams to facilitate safe and effective transition of care between services in order to provide seamless support for patients.

Facilitates communication between different clinical services to provide speedy service for patients with suspected cancer.

Document and monitor all aspects of care coordination and service delivery, supporting data collection for audit.

To ensure that high standards of documentation and care planning for patients are maintained.

Ensure accurate data is rapidly captured and recorded to meet the standards of the management of patients, and to fulfil the cancer information requirements.

Create databases to collect and monitor information, maintain and update.
