South Tyneside Council are recruiting for an Adult Social Worker to join their Learning Disabilities Team.


Overall Objectives of the Post:


Our Adults and Integrated Care Service is responsible for a range of services and activities that support people with social care and health needs, including carers.

The approach used by the service puts more focus on supporting people to remain independentand to live better lives.


Our Let's Talk Together approach, based on 3 conversations, puts the person at the centre of their decision making, as experts in their own lives.

Our staff enable people to explore the networks around them that can support:


  • Their wellbeing
  • Maintain and develop their independence
  • Prevent, reduce and delay needs
  • Connect them to their community
  • Build on their strengths and assets


Our aim is to work with all our health and social care partners as part of a multi-disciplinary community team and deliver Practice to be Proud of to support the people of South Tyneside to Live Better Lives.


To enable us to do this, we are changing our practice to have conversations to find out what really matters to the people we work with and we are getting to know what there is in our communities, so we can link people with the right level of support.

SocialWorkers empower individuals, their families, carers and communities to lead fulfilling, independent lives.

Using our Let's Talk Together strengths-based approach you will work collaboratively with people with social care needs to formulate support plans thatmaximise abilities, enabling individuals to make informed choices and work towards their identified goal.

You will contribute to the development, performance, quality assurance and continuous improvement of Adult and Integrated Care Services.

Ensuring servicesare responsive to the local and national landscape and contribute to the Council's aims and objectives within an overall framework that safeguards adults, promoting health, independence and wellbeing.
